Step 2 – Complete the Pet Insurance Claim Form
Our claim form is easy to understand and is divided into 3 simple parts:
- Part 1: General Information – This section helps us to identify you and the pet that you are filing the claim about.
- Part 2: Diagnosis/Symptom Information – This part of the claim form is extremely crucial since it tells us why you are filing the claim.
- Part 3: Pet Owner Declaration – After reading the declaration on the claim form, we need your signature to process your claim.

Completed claims forms can be faxed, mailed or emailed to us along with an itemized invoice to the contact information listed on your claim form. Please choose only one method to submit your claims, as duplicate claims submissions can delay claim processing.
Once you have submitted your claims, do not forget that you can track the status of your claim online 24/7 at PetPremium’s Member Center. We will notify you via email upon receipt of your claim, if we need more information to process it, and when your claim has been finalized.
